I have a "tea etiquette" book, i'll have to see if the "pinky up" refinement is mentioned there.
I might be partial to "chew with your mouth closed" one. and its interesting how, at least seemingly, putting your elbows on the table some places is bad form, and other places maybe okay.

(I actually had some hot chocolate a few days ago. was riding to the library, got drenched in the rain, and the library had a fan going inside for goodness sake!)
Don't go with Swiss Miss myself... tall micro-safe mug (the sturdy ones from Dollar Tree) with milk & water in equal measure heated at 1100 watts in the microwave for 4-minutes, then 2-tbs Nestlé cooking cocoa and 2-tbs of Nestlé hot chocolate added together, stirred well... I think I'll make some now :)
