What was the significance of adding the screen with the prostitute? What did it represent? Was it needed?

I'm assuming you mean "scene" not "screen".
The scene is included to show:
A Winston had sexual desires that could not be catered to by boyfriend/girlfriend relationships, since they were banned.
B Sex is dirty.
I'm pretty sure Orwell had some serious hang-ups in the sex department himself.

I think the main reason winston was with the prostitute is that he wanted to have a sense of rebellion against the government. Since having sex is a illegal in their world. Also it shows that winston is somewhat greedy to have sex because one: he had sex with a prostitute. two: he had sex with an OLD prostitute. and three: he hasn't had sex for a lond timwe since his wife disapeared
