After Winston is captured when O'Brien comes and says "they got me a long time ago" does that mean that O'Brien was always with the Thought Police or was he actually captured and forced to trick Winston and Julia?

Around the time with the men with white coats,Winston isn't dead is he?

I don't think he's the head, because there doesn't appear to be a single head in any branch of the Inner Party. It's a collective oligarchy, something which has never existed. O'Brien will be very senior thanks to his age, but Orwell doesn't give much insight into the structure of the Inner Party, although O'Brien's admission that he co-wrote The Book says that if there's a ruling elite within the Inner Party, he will certainly be in it.
