I don't know whether this post should be in this section, so please forgive if it is not.

I am a mature student that hasn't written an essay in 20 years so have forgotten a few things. i have written the piece I need but am unsure whether a) I need to out the question in with the essay at the top or if the essay just begins with my description, b) do i need to put my name on each page or does this just go onto the separate piece i hand in given to me from my college.

If anybody could help me would be great I have tried to find these answers online but to no avail.
Thanks

Just write and revise and present it. We will read and of course some feedback will crop up.

You don't need to put the essay question at the top of your essay. This (the essay question) should be summarized in your thesis statement (introduction).

I think the MLA format requires you to put your last name on each page next to the page number. This was included in an example of a paper that used MLA format, so I just assumed that it should be there.

Insert a header that reads "(Your last name) (page #)" 1/2" from the top of the paper.

If it's supposed to be in MLA format then you don't need the question attached to it.
