Lady Macbeth definitely wears the pants in that relationship. It can be safely assumed that she lost a child, and that she detests being a woman because of this. She doesn't want to feel, and is completely ruthless as a result. She feels like she is a failure of a woman, I think.

As for historical context... I don't know. I never studied that time period very extensively.
