I'm thinking that in the scene where Juliet's Mother and the Nurse talk about Juliet's youth that maybe her mother calls the nurse by her name.

Maybe... it's been awhile since I've read that play.

ooh good question, I know one of the servants is named Angelica (cause that's my name and I payatttention to that kind of thing) but I am not sure if it is THE nurse or just some other nursemaid. Although the line is spoken by Juliet's mother so it could be to the nurse.
